The U.S. Consumer Product Safety Commission (CPSC) announced that the bottles can pose “serious impact and laceration hazards” because the lid may forcefully eject when opened, particularly if food, carbonated drinks, or perishable liquids have been stored inside for a period of time.
Walmart has received three injury reports linked to this product. In all cases, the users were struck in the face by the ejected lid. Alarmingly, two of the individuals reportedly sustained permanent vision loss due to being hit in the eye. The CPSC emphasized that the issue arises specifically from pressure buildup inside the bottle, which can result in violent lid detachment upon opening.

The water bottles were manufactured in China and imported by Olympia Tools International Inc., a company based in California. Walmart is the exclusive retailer of this product. The defective bottles can be identified by the model number 83-662, although this number only appears on the product’s packaging and not on the bottle itself.
Physically, the product is a silver stainless-steel bottle with a black, one-piece screwcap lid, measuring approximately 4.41 x 4.41 x 11.5 inches. The Ozark Trail logo is visibly embedded on the side of the bottle. Consumers who purchased the item are advised to stop using it immediately and follow the recall instructions provided by the manufacturer or Walmart to avoid potential injury.
